When placing your order, please include your name, the subject's name, the approximate date of your original Keepsake order, and the Kit ID# or Order number in the Order Notes section at checkout; this ensures your Replica is a perfect match (Not sure about some of this information? Fill out as much as you can; sometimes limited information is enough. If I need to know more, I'll contact you using the email address you used for your purchase). Then complete the Impression Form to select your customization options, such as glass color, inscription, and whether you want it made for hanging.
